Forms & FAQs


First time patient forms

Please call for appointment availability.  Once your appointment is on the books, please complete these forms ahead of time and email them to me or bring a completed hard copy to the first appointment.



how much does it cost?

My fee is $165 for individual counseling (1 hour) and $245 for couples counseling (1.5 hours). I charge a cancellation fee for appointments canceled or missed with less than twenty-four hours of notice. If you need help, please don't allow cost to stop you. I will always do what I can to assist in finding an appropriate therapist in your price range.


For those patients who are not local to the Los Angeles office, I provide psychotherapy via video-conferencing through a secure, HIPAA-compliant platform. Please contact me for further information or to set up a virtual appointment.

do you accept insurance?

I have chosen to accept some insurance plans, but please call or email me to confirm whether your insurance will cover your treatment.  The specifics of insurance plans can sometimes be tricky and do periodically change.  I will happily confirm for you whether your insurance can be utilized for treatment with me.

are you licensed?

I am licensed by the California Board of Behavioral Science as a Marriage Family and Child Therapist (License Number MFT32647). I have a masters degree and completed over 3,000 hours of clinical experience prior to taking my licensing board exams in 1995.

what about medication?

In some cases, a combination of medication and therapy is the right course of action. While I do not prescribe medication, I will refer you to a trusted psychiatrist or I will work with your medical doctor to determine what seems best for you. The long-term solution to mental emotional problems and the pain they cause cannot often be solved solely by medication. However, an integrated approach combining therapy with medication frequently yields excellent and lasting results.
